Why does my google docs form appears all white in my website?

I'm using google sites for a website of my own and it happens that when I insert a form in a page, it will show completely white (except for the title).





I tried changing the permissions, but it won't solve the problem.


I also erased my cookies and cache, but it won't solve the problem either.





Can you help me?
